Question
MP/H Rules/Histology--Prostate: Is adenocarcinoma with squamous differentiation coded as 8570/3 (per SINQ 20021062) and reported as a new primary? See discussion.

Discussion
A patient is diagnosed with adenocarcinoma of the prostate in 2007. He is followed and treated with hormone therapy for 4 years. Then in 2011, he developed left hydronephresis and underwent a TURP The pathology report stated "INVASIVE PROSTATIC ADENOCARCINOMA WITH EXTENSIVE SQUAMOUS DIFFERENTIATION. GLEASON SCORE: 4 + 5 = 9. PERINEURAL INVASION: POSITIVE."

Answer
This is a single primary according to rule M3. Report only one adenocarcinoma (any type) of the prostate per patient per lifetime.
