Question
Reportability--Heme & Lymphoid Neoplasms: I have seen an increase in the coding of essential thrombocytopenia. Many times it has been coded based on blood counts. I have also seen discharge summaries that say thrombocytosis and they get coded to essential thrombocytopenia. I need clarification on which cases should be reported.

Discussion

Answer
The reportable diagnoses are essential thrombocythemia (9962/3), essential thrombocytosis (9962/3), and refractory thrombocytopenia (9992/3). Thrombocytopenia (NOS) and essential thrombocytopenia are not reportable.

Thrombocytosis by itself is not reportable. However, essential thrombocytosis is one of the alternate names for 9962/3, so essential thrombocytosis is reportable.

The diagnosis of essential thrombocythemia is based on blood counts, but is usually a diagnosis made by excluding other myelodysplastic disorders. See also in the Hematopoietic Manual Appendix F: Non-Reportable List for Hematopoietic Diseases.
